Possibilities and limits from the viewpoint of social medicine

According to the provisions of German social law, obesity is regarded as a disease if complications of overweight plus an increased body mass index are present or if obesity is extreme (BMI > 99.5th percentile).
Long-term effectiveness of treatment programmes for obese children and adolescents has not been reliably demonstrated so far, however. Controlled clinical trials including aspects of research into the health systems are needed. The paper presents criteria to be fulfilled by treatment programmes which are eligible for such trials.
